What Is a Drivability Form?

A drivability form is a diagnostic questionnaire that documents what your vehicle is doing and the conditions under which the problem occurs.

It can help describe starting problems, rough idling, stalling, hesitation, lack of power, unusual noises, vibrations, warning lights, poor fuel mileage, smells, leaks, or concerns that only occur under certain driving conditions.

The form does not replace diagnostic testing. It provides useful details and gives the technician a clearer starting point when evaluating the vehicle.
